Event Registration

Medical Release: By signing my name in the box below, I agree that I am the parent or guardian of the aforementioned participant, or am the participant myself, and am over the age of 18, and agree to the following conditions:

I give my consent to BattleCreek Church, including any Staff, Medical Team, Safety Team, and Volunteers participating in the aforementioned event, to administer to the participant any medical treatment, including medications, they deem necessary during the participant's participation at the aforementioned event.

I authorize BattleCreek Church, including any Staff, Medical Team, Safety Team, and Volunteers to obtain for the participant any outside or off-site medical and/or surgical treatment they deem necessary and authorize the treating physician (chosen by BattleCreek Church) and his or her medical staff to carry out any medical and/or surgical treatment in accordance with the treating physician's diagnosis.

I give BattleCreek, including Staff, Medical Team, Safety Team, and Volunteers permission to transport the participant to obtain medical treatment.

I understand that I will be notified, as soon as practicable, of any medical treatment administered to the participant.

I understand that I will be responsible for all costs associated with any such medical treatment.

I hereby agree to hold BattleCreek Church, including its representatives, agents, employees, medical team, safety team, and volunteers, free of any and all liability, actions, causes of actions, claims, expenses and damages on account of injury to the participant or property; even injury resulting in death, which may arise in connection with the aforementioned event or any other associated activities, including the administration of medical treatment.

Photo/Video Release: By signing my name in the box below, I agree that I am over the age of 18 and am legally able to give consent for the aforementioned participant. I understand and will allow photos and videos of the participant taken while at the event to be used in any of BattleCreek Church’s publications. I also understand that the publication of these photographs may be accomplished electronically via the Internet and that after publication BattleCreek Church will be unable to prevent persons from gaining access to the Internet, copying my photographs and video, and subsequently using, altering, or republishing it without my consent. I waive any claim for damages against BattleCreek Church from unauthorized use, alteration, or republication of photographs and video by third parties accessing the Internet. No compensation will be offered for the use of the participant's image or likeness.

Event Participation and Release: By signing my name in the box below, I agree that I am the parent or guardian of the aforementioned participant, or am the participant myself, and am over the age of 18, and agree to the following conditions: 

I give my consent to participation in the aforementioned event.

Participant is physically and mentally capable of participating in the activity.

Participant understands the importance of safety instructions and agrees to follow all posted rules and verbal instructions provided during the bunk bed build.

Participant will wear safety glasses at all times.

Participant will wear proper attire at all times; no loose fitting or dangling clothing or jewelry, no open-toed shoes. 

Participant will not eat or drink while in the workshop area or while performing any build task outside of the workshop area.

Participant will not operate or use any tools or machinery without the presence of at least 2 adults.

Participant will not engage in horseplay or in any way distract others while they are performing a build task or operating tools or machinery.

I acknowledge there are many inherent risks, dangers, and hazards associated with serving during a bunk bed build including but not limited to (1) use and operation of tools, machinery, and other equipment commonly found in a woodshop, (2) lifting, moving, placing precut materials such as lumber for assembly, (3) lifting, moving, handling mattresses and bedding.  

I understand that there are risks involved in participating in this activity, and I hereby agree to hold BattleCreek Church, including its representatives, agents, employees, medical team, safety team, and volunteers, free of any and all liability, actions, causes of actions, claims, expenses and damages on account of injury to the participant or property; even injury resulting in death, which may arise in connection with the aforementioned event or any other associated activities.
